I have attached a copy for you to look at and read. https://www.neighbourhoodalert.co.uk/Attachment/198531/136907/20311/20311_Model_of_Care_for_29th_Jan_workshop___The_New_Vision_of_Care.pdf

Invitation
You are invited to a public workshop planned for tomorrow - Friday 29 January 2016 from 1.00pm – 3.30pm at: SportsAble, Braywick Sports Ground, Maidenhead SL6 1BN
This is being organized by East Berkshire NHS and the Local Authorities as part of the work, to design a new model of care, for people with complex health needs – those with more than one long term condition that need support and care, from different parts of the NHS and social care.
There will be opportunities to hear more about what changes are planned, to ask questions and share your views.  You can book your place by emailing communications.eastberksccgs@nhs.net
If you are interested but can’t make the workshop tomorrow, you can find out more on one of the Clinical Commissioning Group websites (below) and there will be future opportunities to get involved.
